Strengthening Community -based Care Model for Detection, Registration, Referral and Care for hypertension and T2DM patients in Siaya and Kisumu counties in Lake Victoria Basin, Kenya
SHORT BIO
Sylvester is a practicing Clinician in Kenya with a background in Clinical Medicine, Surgery and Community Health. I have Msc. in Epidemiology. Am highly passionate about Epidemiological studies, implementation and Community-Based interventional Studies and in Clinical trials. My prior experience is in implementation of Clinical trials and Community-Based patient registries for over ten years in areas of Maternal and Child Health. These were internationally funded projects through research institutions in Kenya.
Have grown in the region with families seeking NCD services late. It was during my Msc. when I assessed the primary care model in management of NCDs in Western, Kenya that the gap in NCD Registries became clear. This motivated my journey towards establishing A digital NCD registry to assist in Screening, Registration, Referral, follow-up and Management of NCD patients in Lake Victoria Basin. The registry to be utilized both at Community and Primary Healthcare facilities.
Project description.
Hypertension and diabetes are increasingly becoming a significant public health issue in East Africa. Leveraging community-based strategies will boost access to NCD services and improve disease management.
My PhD is hinged on establishing a Digital NCD registry and strengthening Community-Based care Models for Detection, Registration, Referral, follow-up and Management of hypertension and type 2 diabetes within the region. Am working towards utilization of digital health technologies like Digital NCD Registries, geo-localizing NCD burden to optimize service delivery, implementation of digital NCD registry and Mobile application to facilitate community cum primary healthcare level data collection and education.
